ENROLLMENT PROCEDURE



I - FRESHMEN/ TRANSFEREES

STEP 1. Present the following requirements to the Admissions Office located at Rm. 209 of PR Hall.

FRESHMEN REQUIREMENTS
  • Original Copy of High School Report Card (Form 138)
  • Original Copy of Certificate of Good Moral Character
  • 4pcs. Colored ID Pictures ( 1 1/2 x 1 1/2 )
  • Photocopy of Birth Certificate ( NSO )

TRANSFEREE REQUIREMENTS
  • Transfer credentials from college last attended
  • Certified copy of student's grades
  • Certificate of Good Moral Character
  • 4 pcs. Colored ID Pictures ( 1 1/2 x 1 1/2 )
  • Photocopy of Birth Certificate ( NSO )

Note: Applicants for Scholarship must proceed to the Office of the Scholarship committee located at the Guidance Office, 2nd floor of PR Hall before securing Pre-Registration Forms.

STEP 2. Pay testing fee of Php. 200.00 to the Cashier located at 1st floor of HR Hall.
Note: Those who are exempted from taking the entrance examination or have already passed the entrance examination may proceed to Step 4.

STEP 3. Proceed to the Guidance Office located at Rm. 210 of the PR Hall for the entrance examination.

STEP 4. Secure Pre-Registration Forms at the Admission Office.



II - ONE - STOP ENROLLMENT

STEP 1. Pay down payment to Cashier located at 1st floor of HR Hall
  • BS Nursing - Php 4600.00
  • Other programs - Php 4200.00

STEP 2. Proceed to the CCP Main Library, located at the 3rd floor of the Antonino Hall, and present accomplished Application and Pre-Registration Forms of the program adviser for subject and schedule approval.

STEP 3. Claim the Adviser's Slip at the Computer Section located leftmost door of the CCP Main Library.

STEP 4. Proceed to the PE Department and NSTP Section located at the 2nd floor of the PR Hall and Adviser's Slip for validation.

STEP 5. Proceed to the Bookstore located at the 1st floor of the PR Hall for ID picture taking.

STEP 6. Proceed to Window 1 or 2 of the Registrar's Office, located at the 2nd floor of the Antonino Hall, for enrollment validation.



FOREIGN STUDENTS
Documentary Requirements

  1. Transcript of Records / Scholastic Records.
  2. Course Description or its equivalent duly signed by the Registrar of the former school (for transfer students only).
  3. A notarized Affidavit of Support including bank statements or notarized grant for institutional scholars, covering the expenses for the student's accommodation and subsistence, as well as school dues and other incidental expenses.
  4. Medical Health Certificate issued by an authorized physician but not limited to standard-size chest X-ray, HIV, Hepatitis B clearance.
  5. Police Clearance Certificate issued by National Police Authorities in the studen's country of origin or residence.
  6. Photocopy of the passport showing student-applicant's birth date and birthplace.
  7. Five (5) copies of the Student's Personal History Statement (PHS) duly signed by the student.
  8. Five (5) 2x2 inches photograph with plane white background taken not more than six months prior to the submission.
  9. Twelve (12) pieces of passport size photograph.


Note: Document requirements 1 to 5 must be properly and separately authenticated by the Philippine Foreign Service Post located in the student-applicant's country of origin or legal residence.
